Bowling Tournament Registration Form
REGISTRATION OPTIONS:
PSI Members $400
• Cost is per team, 5 players per team
• Must include one PSI member
• Includes 2 drinks per person (beer or wine), delicious appetizers, shoe rental, bowling, Ping Pong and billiards
Non PSI Members $555
• Cost is per team, 5 players per team
• Does not require a PSI member
• Includes 2 drinks per person (beer or wine), delicious appetizers, shoe rental, bowling, Ping Pong and billiards
Single Bowler $105
• Cost is per person
• PSI will match single bowlers with others to form a team
• Includes 2 drinks (beer or wine), delicious appetizers, shoe rental, bowling, Ping Pong and billiards
Non-Bowler $75
• Cost is to attend the event and not bowl
• Donation entitles you to all the nights’ other fun
• Includes 2 drinks (beer or wine), delicious appetizers, Ping Pong and billiards
**Each team is responsible for filling their team roster, naming the team and designing and procuring their team’s bowling shirts.
